Providence Allergy and Immunology Centre (the "Clinic") is committed to protecting the privacy, confidentiality, and security of your personal information, in compliance with the Personal Information Protection Act (PIPA) of British Columbia, the College of Physicians and Surgeons of BC (CPSBC) guidelines, and best practices recommended by Doctors of BC.

2. Scope
This policy applies to all staff, contractors, and affiliated healthcare providers who have access to personal information collected, used, or disclosed by the Clinic.

3. Collection of Personal Information
We collect personal and health information to:
- Provide medical care and treatment
- Communicate with patients and other healthcare providers
- Schedule appointments and manage billing

- Account updates, clinic-related information, notifications about purchases or appointment management, responses to customer questions, conversational messaging, and notifications about the delivery status of products or services.
- Comply with legal and regulatory obligations

We collect information directly from you or, with your consent, from other providers (e.g., family physicians, specialists, pharmacists).

4. Types of Information Collected
- Full name, date of birth, address, contact information
- Provincial health number and insurance details
- Medical history, referral notes, treatment records, test results
- Emergency contact and consent forms

Your information is used strictly for purposes related to your care or as otherwise permitted or required by law. We may disclose your information:
- To other healthcare professionals involved in your care
- To health authorities or insurers for billing or coverage verification

- When required by law (e.g., public health reporting, court orders)

Your consent is implied when you seek care at our Clinic. You may withdraw or limit your consent at any time by contacting us, except where restricted by law or where consent is not required.

We store your information in both paper and electronic formats, with safeguards including:
- Password-protected electronic medical records (EMRs)
- Physical security (locked offices, restricted areas)
- Confidentiality agreements for staff

You have the right to access your personal health information, with limited exceptions. You may also request corrections if you believe information is inaccurate or incomplete.

9. Retention and Destruction
We retain personal information as required by professional standards and applicable laws. When no longer needed, records are securely destroyed (e.g., shredding of paper, certified digital deletion).

This policy may be reviewed and updated periodically to reflect changes in law or practice. The most recent version will always be posted in the clinic and on our website at www.providenceallergy.ca

13. Data Sharing
- Customer data is not shared with third parties for promotional or marketing purposes.
- Mobile opt-in and consent are never shared with anyone for any purpose. Any information sharing that may be mentioned elsewhere in this policy excludes mobile opt-in data.

14. Messaging Terms and Conditions
By providing your phone number and agreeing to receive texts, you consent to receive text messages from Providence Allergy and Immunology Centre, from 778-726-7006 regarding account notification, customer care, delivery notification. Consent is not a condition of purchase.
